Occupational Health and Safety

The University of Cincinnati has an Occupational Health Program for all employees managed by University Health Services. This will serve as a reminder concerning procedures to follow in the event that you sustain a work-related injury or illness while at work.

If the problem is an emergency, call 911 and ask to go to The University Hospital. If the problem is not urgent and it occurs during UC working hours, go to the University Health Services clinic on the first floor of Holmes (513-584-4457) or the University Health Services clinic on the second floor of Scioto (513-556-2564). During evenings and weekends, go to The University Hospital.

As soon as possible, fill out an "Initial Report on Work-Related Injury of Illness" report (FORM A-1352(a), have your supervisor sign it, and FAX it to Environmental Health & Safety (556-4981). Depending on your Department's policy, you may be required to report to University Health Services before you return to work.
